Mailchimp Form

How do you edit the Mailchimp signup form? I want to put in an extra field, but not sure where to access the default form to add a field.

Please go to front-end and then open panpan.php find the line 204 and put your fied like the screenshot bellow

Thanks! I think I was able to figure it out. I put in my form in the Mailchimp box instead.... it seemed to work and I subscribed myself to the mailchimp list.
